I have exactly the same problem, only with an nVidia GeForce FX5200. Thusfar, I have tried new drivers, old drivers, omega drivers, cooling solutions, reinstalling Windows, scanning my disk, and turning down the settings all the way, to no effect whatsoever. It crashes every bit as much.

I've been having this same problem but it wasn't the video card, it was my memory. I was running 2-2-2-5 timings (stock for my ram) on a 1T command rate. I eased back the timings and set a 2T command rate and the problem has disappeared.

I don't know if it's the same problem with you but this definitely fixed it for me (the white boxes are gone aswell).
